PRACTICAL GUIDE

Getting care in Japan: what it really costs

Why the same treatment can cost very different amounts, and how to compare quotes.

In short

  • Japan is one of the most medically advanced countries, and care here is often cheaper and faster than in the US, Canada or the UK.
  • Without Japanese insurance you pay the full standard price, and some hospitals add a markup for foreign visitors.
  • You need a registered agency for major surgery, rare diseases and some special cases at big hospitals, but it's usually optional for clinic visits, cosmetic surgery and often health checkups.
  • Compare written quotes from the hospital and the agency before you choose.

  • Faster 18 wks is Canada's median wait for an MRI. In Japan it's often 1 to 2 weeks, and you can book many specialist clinics directly. Fraser Institute, 2025

The costsWhere the extra cost comes from

No national health insurance

In Japan, insured patients usually pay 30% of the bill and their insurance pays the rest. Visitors aren't covered unless they live here and join the national system, so they pay 100%. MHLW (Japanese)

If you're coming to Japan for treatment, travel insurance usually won't help. Most policies only cover sudden illness or injury during a trip. Check your policy before you go.

The hospital's foreign-visitor rate

Without Japanese insurance, each hospital sets its own price. Most charge the standard rate, but about 7 in 10 hospitals certified for international patients charge more, often over 2×, and since April 2026 some may charge up to 3×. MHLW 2023 survey (Japanese) · MHLW notice, 2026 (Japanese)

The agency's fee

You need an agency for major surgery, rare diseases and some special cases at big hospitals. For clinic visits, cosmetic surgery and many health checkups it's optional, and it's up to the clinic.

Registered agencies arrange your hospital, visa papers and payment. Hospitals that use this system often say the patient pays the agency's fee. Example hospital guidance

In our experience working with agencies, many charge a percentage of the hospital bill, often around 10% to 30%. Some charge a flat fee instead. There's no official figure, so always ask for the fee in writing.

Other costs

Interpreting, report translation, flights, hotels and any follow-up visits. Some packages include interpreting; others charge separately.

What this hospital charges foreign visitors With valid Japanese health insurance, you pay 30%. Without it, you pay at least 100%, and some hospitals add a markup.
Language help You can hire an interpreter yourself, or use an agency. An agency arranges the hospital and paperwork, but it still uses an interpreter, so you pay the agency fee plus the interpreter fee.
Interpreters usually charge about ¥2,000 to ¥10,000 per hour. With an agency, this is added on top of the agency fee.

You’d pay about ¥65,000 Complete total ¥65,000

  • Standard price ¥30,000
  • Foreign-visitor markup ¥30,000
  • Interpreter ¥5,000

Same treatment, different choices: from ¥30,000 (100% rate, no help) to ¥126,000 (300% rate, 40% agency), before interpreter and travel.

This is a rough estimate. Real prices change with the hospital, the agency, how long you stay, how many hours you need an interpreter and what they charge, and which tests the doctor orders. Ask each hospital and agency for a written quote.

TipsHow to keep costs fair

  • Ask the hospital's rate. Ask your agency what the hospital charges foreign visitors per point, or how its price compares with the standard price.
  • Get the agency fee in writing. Ask whether it's a percentage or a flat fee, and what it includes.
  • Compare two or three quotes for the same treatment.
  • Check what's included: interpreting, translation, transport, follow-up.
  • Not sure if a quote is fair? Ask a question. There is no fee to ask.
